Scope holding sign (SHS)  +          LES activation

 It is a new field of endoscopic diagnosis which allows us to understand the motor function of the digestive tract through endoscopic imaging. Originally, it was believed that the lower esophageal sphincter (LES) could not be observed by endoscopy, but it has been clarified that contraction of the LES can be visually captured by inflating the stomach under inverting view at gastric fundus. This is what we call "scope holding sign (SHS)." 

胃内で送気すると、LESが収縮して、胃内容の逆流を防止させようとします。このLESの収縮は、胃内での反転視により動的に観察することができます。これをScope holding sign (SHS)と呼んでいます。

LES contraction can not be observed in forward view.    

LES relaxation

When insufflation occurs in the esophagus, the LES relaxes, so there is no endoscopic observation of LES contraction. This is because the LES works as a one way valve to pass food through the esophagus to the stomach.

食道で送気した場合は、LESは弛緩しますので、内視鏡でLESの収縮をみることはありません。これは、LESが食道から胃に食べ物を通す一方弁として働いているからです。

No LES activation: GERD 

In individuals with severe reflux esophagitis, the LES often fails to contract and reflux of gastric juice cannot be stopped. Laparoscopic anti-reflux surgery is recommended for these patients.

逆流性食道炎の強い方では、しばしばLESが収縮せず、胃液の逆流が止められない状況になっています。このような方では、腹腔鏡による逆流防止手術が奨められます。
